Course Overview

Understanding systems and their failures is crucial for improving processes. By examining problems both before and after they occur, we can identify underlying issues and prevent future occurrences. Systems can fail due to various reasons, and recognizing the correlation between causal factors and resultant failures is essential. This asynchronous course will introduce causal factors, from physical symptoms to latent organizational gaps, and help you learn the process to uncover the root causes of failures. Root Cause Analysis (RCA) tools such as Pareto, Fishbone, 5 Why, A3, Fault Tree Analysis, and Cause Mapping provide will be defined. Each tool offers a unique approach to identifying and mitigating problems, contributing to a more robust problem-solving framework.

These tools are essential components of an effective Asset Management System, helping organizations implement a culture of continuous improvement. By applying learning about the tools and how to apply them, you will learn how to develop strategies for preventing recurrence, ensuring a proactive approach to asset management and process improvement.

Course Objectives:

  • Understand systems thinking and causal factors.
  • Identify various RCA tools and their differences.
  • Understand which tool to apply in various scenarios.
  • Identify the requirements necessary for continuous improvement culture.
